Harmony CCS Limited has been fined £50,000 following the death of Mary ‘Pat’ Thompson, an elderly resident, who fell while being cared for by a single carer despite her care plan indicating the need for two. The Health and Safety Executive for Northern Ireland highlighted systemic failures in the company’s procedures for assessing and training staff, which contributed to the tragic incident at Rose Lodge Nursing Home in Lisburn.
